NY State Health Exam & Screening Requirements
Please read carefully.
Only the approved NYS School Health Examination Form may be used to document health exams to provide consistency across the state.
All immunization records must comply with NY mandates.
Please submit your child's most up-to-date physical exam and immunization record by August 20th for state compliance review. You will receive email reminders to scan and attach the forms in TADS Enrollment.
Immunization Record Requirements:- Specific Requirements are included on the NYS Immunization Form.
- Please note that Kindergarten vaccine mandates are not age-relevant. Regardless of your child's age, all students entering Kindergarten must comply with NYS mandates.
- Students not in compliance with NYS immunization mandates within the first 2 weeks of school are required by law to be excluded from school attendance.
Health Exam Requirements:- For the School year 25-26, exams must be dated after 9/1/2024.
- For the School year 26-27, exams must be dated after 9/1/2025.
- Form must be submitted before the start of the school year.
- New York State requires that a health exam form be on file at the Health Office for all new admissions and for grades Pre K or K, 1, 3, 5, 7
- Vision screening, including color perception, distance, and near vision acuity, is required for all new admissions and for grades Pre K or K, 1, 3, 5, and 7.
- Hearing screening utilizing pure tone testing will be required for all new admissions and for grades Pre K or K, 1, 3, 5, and 7.
- Scoliosis screening is required in grades 5 and 7 for girls.
- You may opt your child out of any screenings, please contact the school nurse for more information

The school nurse may administer medications to students during the school day with a written prescription from a physician, written permission from the parent/guardian, and the medication in the original or pharmacy-labeled container. Please complete any applicable form(s) below and send to the school nurse.

Dental ExamNew York State law permits schools to request a dental examination in the following grades: school entry, K, 1, 3, 5, 7. Your child may have a dental check-up during this school year to assess his/her fitness to attend school.
